  3. 1A- Since originally is preceded by department, I took it as just D. It turns out that 'originally' is for hospital.

  4. 'Originally' is not for hospital. It indicates that DEPT goes ahead of H.
    That is, the order is not
    H DEPT
    but
    DEPT H, where 'dept' appears 'originally', at the beginning.
