The answer to 4d reminds me of a book written by Alan Connor: Two Girls, One on Each Knee (The Puzzling, Playful World of the Crossword). The title itself is cryptic clue with 'one on each knee' as definition with the answer as PATELLA {PAT}{ELLA}
