Dr RKE's TalePiece

Doctor’s Day Tale

Viranna Nayak was huffing and puffing on the TREADMILL in the STATE-OF-THE-ART heart hospital in Bengaluru. He had only come to GET a PHYSICAL examination done with his cardiologist, Dr.Mahalingam (MALI for short). The specialist had OBSERVEd a worrying change in Nayak’s ECG and had ordered the treadmill test.

Viranna Nayak, an agriculturist from RAICHUR, had UNEARTHEd a high-YIELD variety of an OILIEST groundnut some years ago, that had SHOT him into fame. He no longer needed to work in  the fields as he had done while young. The physical inactivity and the oil rich diet made him the IDEAL candidate for a coronary. More than all these factors, it was the stress about his son that caused him the heartache.

Viranna Nayak started feeling EXTREMEly faint as the treadmill was accelerated to the next level. Without any warning, he suddenly collapsed. As he was falling over the EDGE, Ms ANGEL Mary, the alert nurse supervising the treadmill test, stopped the machine and lay his limp body on the couch.  “Jesus, he has no pulse, call Dr.Mali” she shouted and started to give him a cardiac massage. She inserted an ORAL airway and proceeded to give mouth-to-mouth respiration. In the IDEALISTIC set up that this hospital was, it was possible to resuscitate him within 3 minutes and move him directly for a percutaneous coronary intervention to the cath lab in the next 10 minutes. A blocked left main coronary artery was stented and Viranna Nayak sprang back to life magically from the dead.

Dr.Mali, was not the doctor who treats the illness in the patient, but one who treats the patient with an illness. When Viranna left the hospital on 1st July, Dr.Mali told him “shed your NEGATIVISM, take your son to Dr.So-and-so, an endocrinologist, who specializes in transgenders”. “How on earth did he know?” thought Viranna, “I had not told him anything bout my son”. Dr.Mali did not need to be told. One look at the tall boy of 17, the junior Nayak, who was effeminate, still holding a TEDDY BEAR with the ENAMEL on his TOENAILs was enough for him to guess the source of stress for Viranna. The “boy” was a girl trapped in a boy’s body due to a disorder in “his” Y-CHROMOSOME.

  3. The island had a STATE OF THE ART CCTV set up.

Major Shepherd was not an OLDIE and when a rowdy from RAICHUR tried to GET PHYSICAL he was given the HEAVY METAL treatment by him. And the body will never be able to be UNEARTHED in the island.

Some gatherings were IDEAL and ENDEARING for SOCIAL CLIMBERS.

No outsider in the island would be able to OBSERVE the actual going ons since the discipline YIELDED no clue.

And then came about the Ek Phool do MALI altercation.

    What happened next?

    1. My experience with TMT tells me not to recommend it for people above 50. The results may be erratic.
In 2006, I felt breathless climbing up the stairs and again while climbing a small hill for a temple visit. So, I thought it was better to see a cardiologist. He recommended TMT. In the first levels, it was ok but when they increased the speed, the machine gave warning signals and they asked me to stop. AC was on and they asked me whether I had any difficulty. I said it was fine with me. Then they put me in ICCU for an angioplasty, the next day. [I knew later they sent a thin tube through my groin up to the heart area] Although I was on anaesthesia, I could hear the doctor shouting at his staff "why did you bring him here? He is very fine!!"

  13. Prasad,h
comprises both your heart and blood vessels.1 It does so by comparing your circulation at rest with the same measurements taken at maximum exertion.
The Max.exertion is waht I was talking about- whether it is safe for people over, say 60. It may create a problem. There is a level of exercise corresponding to one's age. It may create a situation as indiacted in Dr.RKE's story- the damage may be permanent. It may take time to realise and stop in the middle of a TMT. I just wanted expert opinion.

    1. Technically I am not an expert.
But TMT is not a routine diagnostic test. It's main aim is to FORCE out CAD (does not always but very high success rate). So it's not something to be done unless a doctor suspects CAD.

And then do it under controlled environment with all the contra checked out.

    2. They tend to do it in routine annual health check ups- where there is no immediate expert supervision, if at all there is any.
      Hence my worry.

    3. I am not a cardiologist, but I would not advise it to be done as routine health check-up Paddy. There are other tests available. Having said that, you should go by what your trusted physician advises.

After further debugging found had missed oral. The viva question heavy metal occurred. Music cue led to Tablature. Could oldie be far behind? Looking at the top uncovered Raichur from the state of Dr. Shetty known for State of the Art. Was about to yield but as I got up hurt my toenail. Massaged it till it was so-and-so less painful. Fit enough to get physical and find an angel perhaps.
